It is a non-cancerous condition

The prostate alargement is the correct term to refer to this disease, because the term hyperplasia is a histological term that means benign proliferation of the epithelium and stroma of the prostate gland.

The following factors are the ones involved in the growth of the prostate: advanced age, increased estrogens and some testosterone metabolites, increased activity of 5 alpha-reductase, which is responsible for the transformation of testosterone to dihydrotestosterone.

Despite the decrease in testosterone levels in the elderly, the increase in receptors for dihydrotestosterone induced by estrogens is sufficient to increase the size of the prostate.

Prostate enlargement causes an obstruction around the urethra, blocking the flow of urine causing symptoms called lower urinary tract symptoms which are:

• Feeling like the bladder doesn’t empty completely
• Urinate frequently (<2 hours)
• Intermittent urine
• Urgency
• Push to urinate
• Thin or smaller urine stream

MEDICAL TREATMENT

Medical treatment helps to improve the symptoms of patients, however not all of them stop the growth of the prostate, so in the vast majority of patients after a few years it requires a definitive treatment such as surgery.

A great disadvantage of medical treatment: is a therapy that does not have a defined management time, which leads to higher costs and complications associated with the adverse effects of medications.

Alpha-blockers are effective for the improvement of lower urinary tract symptoms , their function is to relax the smooth muscle of the prostate and the bladder neck, they are well tolerated and their efficacy is maintained 6-12 months after treatment, they do not reduce the size of the prostate or alter the progression of benign prostate growth.

The 5 alpha-reductase inhibitors are another group of drugs used for the benign growth of the prostate, they inhibit the enzyme 5 alpha-reductase which leads to a decrease in the levels of dihydrotestosterone at the prostate level, which in the long term entails to a reduction in the size of the prostate.The improvement of lower urinary symptoms with these drugs is seen in a period of 3 to 6 months.

SURGICAL TREATMENT

It is the most effective method and with which the symptoms resolve more quickly, there are different surgical procedures, we explain some of them below:

Holmium Laser enucleation of the Prostate (HoLEP): It is currently the best method for the treatment of prostate growth, because in a more anatomical way the tissue grown in the prostate is removed.

This technique offers the best short- and long-term results.

This technique is currently performed by a small number of urologists in the world.

Transurethral resection of the prostate (TURP): It is the most popular treatment worldwide, it allows removing the tissue that is causing obstruction, however with this technique the tissue that is left in situ is much larger than in HoLEP, so the patient may require a second intervention 5 -10 years after the first procedure.
