KIDNEY STONES

They are generated by the condensation of small mineral particles in the kidney

which can be oxalate, calcium, uric acid, phosphate, among others. In concentrated urine, the density will be higher, this is due to the fact that a high amount of solutes is found in it as a result of the little fluid intake.

By creating a conducive environment with a suitable Ph, the condensation of these solutes (crystals) begins one by one, until a solid mass of variable size is formed, which can detach and cause an obstruction in the urinary tract that causes severe pain accompanied by sweating and vomiting.

If the stone does not come off it can continue to grow to the point of forming a large stone that occupies the entire interior of the kidney, which is called “Staghorn calculi”.

Can cause

Renal failure condemning the patient to a renal replacement treatment such as hemodialysis, for which its treatment is of vital importance.

DIAGNOSTIC METHODS

Laboratory tests or image diagnosis

Creatinine levels: It is an indicative marker of kidney function, an elevation of this indicates a failure in the functioning of the kidneys.

General urine test: In this test, the urine is analyzed macroscopically and microscopically, when there are stones in the kidneys, this test reflects bleeding (erythrocytes) and / or infection (leukocytes).

Renal ultrasound: It’s a study that’s carried out initially in the office, non-invasive, does not emit radiation, which allows the rapid diagnosis of some renal alteration, stones in the kidneys and / or obstruction of the urinary tract, however on some occasions some images they resemble stones in the kidney that are not, so it is always advisable to perform a tomography for an accurate diagnosis.

Simple abdominal & pelvic CT: It is the ideal and confirmatory method for the diagnosis of stones in the kidney or in the urinary tract. It provides us with the specific information (size and hardness of the stone) to make the appropriate therapeutic decision for the management of kidney stones.

TREATMENTS

The treatment must be individualized for each patient, since each one has a different urinary system, a different stone composition. The choice of treatment depends fundamentally on three factors: • Stone size • Stone hardness • Location of the stone

CARE AND SUPPORT

Drink abundant liquids Do not stop eating dairy If you have endoscopic surgery, you can quickly return to your daily activities Identify the origin of stone formation

MEDICINES

• Non-steroidal analgesics are recommended for pain control, it has been identified that they are very effective against renal colic, the use of opioids is not recommended as they cause a smooth muscle spasm unless strictly necessary. • Alpha Blocker: it is part of the drugs included in expulsive therapy, it works by increasing the size (dilating) the final portion of the ureter to allow the passage of stones. • Antibiotics: they help fight the urinary infection that regularly accompanies lithiasis.

MEDICAL TREATMENT

Semi-rigid Ureteroscopy:

It is used for the treatment of stones in the ureter with the help of a laser equipment, achieving the pulverization of the stone.

Flexible Ureteroscopy:

It is used for the treatment of stones located in the kidney that are smaller than 2 centimeters.

Extracorporeal shock wave lithotripsy:

Through the transmission of sound waves, the fragmentation of small stones in the kidney is attempted, its disadvantage is that the removal of the stones is not always achieved.

Percutaneous Nephrolithotomy:

It is the ideal treatment for stones of 2 centimeters or more that are located in the kidney.

Combined Approach:

It is a novel method for the treatment of kidney stones combining two techniques (flexible ureteroscopy + percutaneous nephrolithotomy) with which we are able to better eliminate the stones in the kidney, reducing the possibility of leaving a stone fragment.

Expulsive Therapy:

It is used for the elimination by means of medicines of stones less than 1 centimeter that are found in the ducts that drain urine into the bladder (ureter).
